what is cirrhosis?
a chronic, progressive disease of the liver characterized by the destruction of hepatocytes
what diagnostic tests are used for cirrhosis?
- U/S
- labs
- CT
- biopsy
can the liver regenerate itself?
it can regenerate, but not normally. This will cause back flow of blood causing varicies.
what are the early stages of cirrhosis?
- loss of appetite
- generalized malaise
- wt loss
- jaundice (aka icterus)
- itching (increase bilirubin salts)
- nausea
- easy bruising
- nosebleeds
what are the later stages of cirrhosis?
- ascites (will increase hypotension causing increase fluid in and)
- peripheral edema
* *both probably bc of low albumin** - vomiting blood
- black stools
- muscle wasting
- spider angiomas
- dilated blood vessels
- fever and chills
- hepatic encephalopathy (increase ammonia crosses BBB= altered LOC)
- hepatorenal syndrome (10% of people decrease flow to kidneys?)
what is the most important cirrhosis treatment?
- REST! best thing for pt
what medications will the pt receive for ammonia level management?
- lactulose- ammonia will be excreted through stool to help decrease cerebral edema give to pt even if their having loose stools
- neomycin- will decrease ammonia levels to decrease cerebral edema
when should an upper respiratory infection be treated?
promptly
should bleeding status be monitored on a pt with cirrhosis?
clotting factors are being affected
should a patient with cirrhosis have an NGT?
yes but be very carful, pt may have varicies that can burst
how should we manage ascites ?
paracentesis, TIPS procedure
HOB should be 45 degrees but may be difficult for pt.
