What is chronic asthma?
Reversible obstructive airways
Symptoms
Coughing (esp night)
SoB
Chest tightness
Wheezing
Maintenance therapy
- SABA
- SABA + Low dose ICS
- SABA + ICS + LTRA
- Add in LABA, salmeterol or formoterol
- Inhaled corticosteroid + LABA (MART)
- Trial LAMA (tiotropium) or m/r theophylline
When would a step-up in treatment be necessary?
Using inhaler or symptomatic 3x a week.
Night time symptoms 1x a week
Asthma attack requiring systemic steroids in the last 2 years.
Refer
Using >1 inhaler a month
- Not well controlled

- 100 - 400 mcg

- 200 - 400 mcg
Mometasone
- 400 mcg
Ciclesonide
- 160 mcg
High dose ICS

- 800 - 2000 mcg

- 1000 mcg
