Q

If a patient is experiencing organophosphate poisoning what should be administered after atropine?

A

Pralidoxime (2-pam)
1-2g over 5-10 min. q 1 hr prn
Note: pralidoxime not indicated for carbamate poisoning (carbamates are insecti- and herbicides that reversibly bind to AchE and spontaneously disassociate, 2-pam can actually lengthen the AchE inactivation)
