Stimulation of bladder tone
SE: sweating, decrease BP, nausea, abdominal pain, diarrhea, bronchospasm
Carbachol
Rarely used, decreased IOP (Miosis), acute ACG, Pigmentary glaucoma, Sx
SE: Bronchial constriction
CI: Uveitis, Undesirable Miosis
Pilocarpine
DOC for IOP reduction in glaucoma tx (emergency), Acute ACG, OAG, Secondary glaucoma SE: Accommodative spasm
CI: pts. under 40 y/o, cataracts, asthma, neovascular & uveitis glaucoma, Hx of retinal detachment
Reversible: Physostigmine
Atony of intestines & bladder by motility, decreased IOP (topical), overdose of atropine, phenothiazine, TCAs
SE: convulsions, bradycardia
Reversible: Pyridostigmine
Myasthenia Gravis
SE: convulsions, bradycardia
Reversible: Neostigmine
Myasthenia Gravis
Reversible: Demecarium
OAG
Reversible: Edrophonium
Dx of Myasthenia Gravis
Does not cross BBB
Reversible: Tacrine, Donazepil, Rivastigmine, Galantamine
Alzheimer’s disease
Irreversible: Echothiopate
Ophthalmic ointment for chronic OAG, POAG, ACG, Accommodative esotropia
Caution: pesticides & insecticides
Irreversible: Pralidoxime
reactivation of acetylcholinesterase but must be used before aging.
