What is Choliangiocarcinoma?
A
An adenocarcinoma that arises anywhere in the biliary tree: intra hepatic bile ducts to common bile ducts.

He where does a cholangiocarcinoma mostly occur?
A
- Ports hepatis
- Distal tumours obstruct the main ducts and present early
- Intra hepatic tumours present late, can be mistaken for hepatocellular carcinoma

In the majority of cases, the cause of cholangiocarcinoma is unknown. in the remainder what conditions are associated with it?
A
- Primary sclerosing cholangitis: with ulcerative colitis
- Congenital fibropolcystic disease
- Biliary cirrhosis: biliary atresia
- Chronic infection: Clonorchis sinensis Far east

What are the clinical symptoms for cholangiocarcinoma?
A
- Jaundice, pruritis (different to biliary cirrhosis)
- Mild epigastric pain
- Anorexia
- Weight loss
- diarrhoea

On examination of a patient with cholangiocarcinoma what are the signs?
A
- Hepatomegaly
- Palpable gall bladder
- Splenomegaly/ascites: rare

What investigations are carried out?
A
- Haematology: anaemia/raised leucocytes
- Biochemistry: raised serum bilirubin, ALK, ¥GT
- Normal alpha-feto-protein
- Faeces: pale/fatty/blood

What diagnostic procedures are carried out?
A
- Ultrasound
- ERCP: Endoscopic retrograde cholangio-pancreatography
- CT/MRI
- Angiography and venography

What is the treatment for Cholangiocarcinoma?
A
- Surgery: removal
