What about a bruise would suggest NAI?

A

Unusual places e.g. chest
Unusual age i.e. too young to have injured themselves
Unusual Patterns
Petechial bruising

Inconsistant story (Either between people or changing over time)

What about a fracture would suggest NAI?

A
  • Rib, spinal or humeral
  • Too young to injure themselves
  • Healing fractures, late presentation or multiple fractures at different stages

How can you spot a NA thermal Injury?

A

Pattern e.g. Iron

Bilateral

Perfect lines (e.g. straight across hand where it’s been held in hot water)

Sparing of certain regions e.g. “kid fell in a too hot bath” but his butt isn’t burned, this implies he was held down in the hot water and his but was against the bottom so didn’t burn

Kid has retinal haemorrhages, cervical spinal injury, rib fractures & finger marks (or any combination of above). What’s this sound like?

A

Shaken baby

  • FInger marks form tight holding
  • Rib fractures from tight holding
  • Cervical spinal injury from shaking neck
  • Retinal haemorrhages don’t appear in many other injuries as it required rapid change of direction to shear the vessels
