CHF (Congestive Heart Failure) Flashcards
Name Cardiac Glycosides and their action.
Digoxin (Lanoxin)
^ contractility (force of contraction) and efficiency of myocardial contraction = ^CO
Nursing considerations for Digoxin
Take apical pulse for 1 min before adm, withhold of it is less than 60beats/min.
Therapeutic level 0.5-2.0ng/ml
Action of ACE inhibitor
Inhibits renin-angiotensin-aldosterone mechanism by blocking conversion of angiotensin I to angiotensin II, which prevents peripheral vasoconstriction and reduces blood volume.
Also used to treat HTN
Memory aid for ACE inhibitor
always end in -pril
Name ACE inhibitor (Angiotensin-converting enzyme)
Lisinopril (zestril, prinivil)
Enalapril (Vasotec)
Nursing consideration for ACE inhibitors
Monitor BP
Peripheral edema
Persistent dry cough is side effect and doesn’t not indicate respiratory disease or infection.
Name PDIs (positive inotrop)
Inamrinone (Inocor)
Milrinone (Primacor)
Action of PDIs
^myocardial contractility
Decreases preload and after-load.
^ CO
Name ARBs (Angiotensin II Receptor Blocker)
Valsartan (Diovan)
Memory aid for ARBs
always end with -sartan
Action of ARBs
Blocks vasoconstriction and aldosterone-secretion effects of angiotensin II at receptor site.
Lowers BP
Decreases risk for heart failure.
Nursing consideration for ARBs
Monitor BP
Monitor for hypotension, especially clients who take diuretics
Avoid salt substitutes because of Potassium
Name Beta Blockers
Carvedilol (Coreg)
Memory aid for Beta Blockers
Always end with -olol or -lol
Action of Beta blockers
Blocks beta1 adrenergic receptor (blocks beta2 in lungs if dose is high enough) found in cardiac muscle.
Decreases HTN
