HMB synthase aka PBG deaminase deficiency
What accumulates in AIP?
5-ALA
Rate limiting step in haem biosyntehsis

Typical presentation of AIP
Trigger: EtOH, drugs, infection, surgery, starvation
- ONLY NEUROVISCERAL SX
- Seizures/sensory loss
- N+V/hypertension/tachycardia/urinary incontinence

Urine ALA and PBG
Appearance of urine in AIP
Port wine
Tx of Acute intermittent porphyria
IV dextrose + haem arginate
two acute porphyria with cutaneous sx
HCP and VP
Hereditary Coproporphyria + Variegate Porphyria
Dx of acute porphyria with cutaneous sx?
Faecal porphyrins
In which non-acute porphyria would you see liver disease?
PCT = Porphyria Cutanea Tarda
Ferrochetalase deficiency –>
EPP = erythropoietic protoporphyria

Uroporphyrinogen decarboxylase
Skin involvement in non-acute porphyria?
Blistering/Pruritis/Oedema after sun exposure
