3
Q
Three sources of information
A
- The patient or watcher
- Medical record and/or patient’s chart
- Laboratory examination and diagnostic test results.

Q
A clinical approach for assessing nutritional status based on patient’s history and physical examination
A
Subjective Global Assessment (SGA)
5
Q
7 components of SGA. 2 components have 3 sub components each.
A
- History obtained through interview
- Clinical/Physical Examination

c. Rating of nutritional assessment - Anthropometric measurements
a. % Ideal Body Weight
b. % Usual Body Weight
c. % Weight loss - Knee height
- Midarm circumference
- Calf circumference
- Arm muscle area
