A

Selective heating —> neocollagenesis in the dermis (dermal regeneration)

Secondary wound healing response begins —> Epidermal regeneration

What is photo rejuvenation

A

Non- traumatic removal of skin surface layers to induce neocollagenesis in the dermis and reduce wrinkles and improve the dermal layers

Following the removal of surface skin layers at a particular depth, the body‘s natural wound healing response begins to regenerate the epidermidis and the underlying wounded skin layers

The new skin layer will be cytological and architecturally resemble a young and more normal healthy skin

Indications

A
Textural skin damage: 
- fine lines 
- large pores 
- elastosis 
- dull skin
Photo rejuvenation improve skin texture by a process of collagen remodelling

Pigmentary skin damage:

  • solar lentigo (sunspots)
  • dyschromia
  • mottled pigmentation
  • melasma (hormonal changes/ sun damage)

Photo rejuvenation is indicated only for epidermal pigmentary damage and not for dermal Nevi (moles)

Treatment protocol

A

Clean & disinfect area/ machine/ piece

Wear gloves & set out protective eye wear

Clean treatment area and cleanse the skin

If hair in the treatment area, shave it

Photograph area

Set parameters

Apply goggles for client & self

If the lesion area is smaller than the tip size the template supplied Should be used to protect the healthy tissue surrounding the lesion

Apply mild pressure to assure full contact

Trigger pulse

Proceed to the next spot, overlapping is not necessary

Two passes on face or hands one pass on neck or chest

Patient is expected to report light stinging sensation

Cool area & refer to post treatment

Immediate tissue response may include the following

A

Textural: light stinging sensation while pulse is emitted
Pigmented lesion: immediate darkening

Warm sensation, slight erythema

Post treatment

A

Immediate cooling
Sun protection (spf 30 at least)
No sun exposure, artificial tanning, avoid dirt/ debris
Cleaning and moisturizing the following days
Irritation/ redness will resolve within 24 hours

What could tanning after treatment do?

A

Tanning after treatment sessions me enhance melanin regeneration which may result in hyper pigmentation

What if adverse effects appear?

A

Treatment should be discontinued until the treatment site has healed

Return for examination after three weeks 

  • if site has healed and results can be observed, select a darker skin type or longer pulse duration and/ or lower Fluence
  • if results cannot be observed, client should wait another 2-4 weeks and return for an additional examination
  • if no results can be observed during second examination it should be determined if the adverse effects were due to the fact that the patients skin was tanned
  • if the adverse effects were not due to tanning, the treatment should be seized until medical condition is further investigated

Treatment schedule

A

Once a month

Skin rejuvenation consists of 4 to 6 treatments depending on the patient’s needs

Allow skin to heal completely before proceeding to the next treatment

How do you map the face for skin rejuvenation

A

Divide the face into four separate areas

How to perform SR

A

Begin on the centre of the forehead working toward the temple without overlapping

Avoid temple area if there is hair in little space

Apply pulse on crows feet then work on orbital bone and on side of nose using the template

Work across cheeks and turn handpiece when you get to the upper lip in order not to singe the lips

Treat jawline and chin and remember to treat the skin with Full Contact always using your hand to manipulate it to cover the sides of the handpiece tip

Treat the neck with one pass avoiding the thyroid in the esophagus

Complete again on other half of face & neck
