What are the 8 requirements needed in order for a vet Rx to be valid?

A
  1. Name, address, tel no , qualification + signature of prescriber.

If sch 2 or 3 CD, then need RCVS REG NO.

  1. Name. + address of owner.
  2. ID and species of the animal and its address
  3. Date. Rx are valid for six months. Rx for Schedule 2, 3 and 4 CDs are valid for 28 days.
  4. Name, quantity, dose and administration instructions of the required medicine.
  5. Any necessary warnings and if relevant the withdrawal period.
  6. Where appropriate, a statement highlighting that the medicine is prescribed under the veterinary Cascade.
  7. If the prescription is repeatable, the number of times it can be repeated.

A

  2. If NOT possible - give vet med with Northern Ireland MA for species + condition indicated.
  3. If NOT possible either- give GB/UK/NI vet med for other species or condition.
  4. If not possible- then GB/NU/UK HUMAN medicine OR

Vet med authorised OUTSIDE uk considered.

5- Last resort- give extemp/special made medicine.
