Steps to evaluate the degree of success in achieving outcomes
- Examine the outcome criteria
- Evaluate the patients actual behavior
- Compare the two
- Judge the agreement between the two
Discontinuing the care plan
Determine the expected outcomes and goals and meet with the patient to discuss if the outcomes have been met
Modifying a care plan
Reassess
Redefine the diagnosis
Determine if the goals and outcomes are still appropriate
Evaluate the interventions ( the appropriateness and the correct application)
