Purpose of the nursing care plan
To coordinate nursing care, promote continuity of care & listing outcome criteria
Student care plans
Useful for learning problem-solving techniques, the nursing process, written communication, organizational skills and helps to apply knowledge from the literature and the classroom in a practice situation
Interdisciplinary care plans
Designed to improve the coordination of all patient therapies and communication among all disciplines
Explain the “handoff” process
It’s a critical time where nurses collaborate and share important information that ensures the continuity of careens prevents errors
Critical pathways are
Patient care management plans that provide the healthcare team with the activities to be done in a sequential order
They deliver timely care at each phase of the process for each patient
Consultations are
Where you seek expertise to identify ways to handle problems in management or planning/implementation
Nurses role in seeking consultation
- Identify the problem area
- Direct to the correct personnel
- Provide relevant information
- Do not prejudice or influence others
- Be available to discuss the findings ( remember that is still your patient and you need to adhere to their needs)
- Incorporate the consultants recommendations
