•outside police agency report if applicable
The Departmental/Unit Commander will provide a Commander’s Endorsement for all departmental collision. The Commander’s Endorsement shall include:
•overview of the investigation
•investigator’s recommendations
•code of conduct violation if applicable
•disciplinary recommendation if applicable
All departmental collision forms are to be completed and sent through Blue Team within ___hours to the IAU Commander (with Division Commander CC’d), via the Chain of Command
