Describe a long term care facility.
Care given for a long time on a daily or ongoing basis.
Define what makes life worth living?
Answer varies from person to person. Always respect how a resident feels about life.
Give two reasons why people are admitted to long term care.
- To live fairly independently and receive support or staff when they need it.
- May have a health condition that requires medical and nursing support.
List the types of care given in a long term care facility.
Rehabilitation, skilled nursing care, continuous supportive care, respite care (family members who normally provide care need break or are away from home), and hospice care.
Define the role of the members of the interdisciplinary health care team. (14)
- Resident- most important person on the team. Every effort must be made to meet their needs and maintain quality of life.
- Resident’s family-provide valuable information to staff. If a resident can no longer make decisions, family usually takes this role.
- Charge nurse- reports to director of nursing. Day-to-day supervising care.
- Nursing staff- largest department. RN, LPN, CNA. Help with daily living, medical treat,nets, medications, and health promotion.
- Medical director- treats medical conditions and directs general medical care.
- Physical therapist- maintain or improve physical abilities.
- Occupational therapist- fine motor skills, maintain or improve the use of their hands and arms.
- Speech therapist- help with residents who have difficulty speaking.
- Admissions coordinator- helps residents through the admission process.
- Social worker- counsel residents and families. Help apply for Medicaid. Arrange home care services after discharge.
- Activities coordinator- plans and directs activities that improve quality of life.
- Dietician- plans and prepares meals.
- Spiritual counselor- gives guidance, coordinates religious services, and counsels families.
- Administrator- manages the facility and directs all staff.
Explain how care is paid for.
State and federal aid (Medicare/Medicaid), private insurance, and personal funds.
Describe the agency that oversees long term care.
The Centers for Medicare and Medicaid Services (CMS) oversees the surveying and certification of long term care facilities. Requires that facilities follow government rules and regulations to receive reimbursement.
Define chronic.
An ongoing illness or condition that does not have a cure, usually has a gradual onset, and lasts for a long time.
Define rehabilitation.
Restoring to former health.
Define restorative care.
Designed to help one return to health and be as independent and functional as possible.
List 4 basic services provided in long term care. Describe all 4.
Medical Care: Resident has care plan written by a physician. Nursing and Rehabilitative Care: Resident needs rehabilitative services after hospitalization. Personal Care: Residents need help with daily activities. Residential Care: Services include supervising residents in secure environment. (Alzheimer’s, pediatrics, HIV-AIDS, etc)
