Chapter 1 Flashcards
P450 inhibitors
SICK
Sodium valproate
Isoniazid
Cimetidine
Ketonazole
P450 inhibitors
Increase drug concentration Sodium valproate isoniazid Cimetidine Ketoconazole Fluconazole Alcohol (acute) / allopurinol Chloramphenicol Erythromycin Sulphonamide Disulfiram Ciprofloxacin Omeprazole Metronidazole Grapefruit
P450 inhibitors and on warfarin
Can cause INR to increase
May need to reduce warfarin dose
P450 inhibitors FACES
Fluconazole alcohol (acute)/allopurinol Chloramphenicol Erythromycin Sulphonamides
P450 inhibitors
Dot com group
Disulfiram Ciprofloxacin Omeprazole Metronidazole Grapefruit
P450 inducers
Effect on drug concentration
Drug to be aware of
Reduces drug concentration
Reduces effectiveness of cocp
P450 inducers
PCBRAS
Phenytoin Carbamazepine Barbiturates Rifampicin Alcohol (chronic) Sulphonylureas
Drug to increase for surgery
Steroids
- may need to double dose
Drugs to stop before surgery
I-LACK-OP
Insulin- variable depending on preparation
Lithium- stop day before
Anticoagulants/anti platelets - variable
COCP/HRT- stop 4 weeks prior to surgery
K sparing diuretics- on day of surgery
Oral hypoglycaemics- variable. But METFORMIN: stop if NBM, can cause lactic acidosis
perindopril and ACEi- day of surgery
Allopurinol
P450 enzyme inhibitor
Xanthine oxidase inhibitor
Used in hyperuricaemia to prevent gout.
Sodium valproate
Prevents rapid action potential firing
Used in seizures, migraines, mania

Isoniazid
Antibiotic
Inhibits mycolic acid synthesis
Part of anti TB therapy
