What is the definition of Nutrition Assessment?
Nutrition Assessment: a comprehensive analysis of a person’s nutritional status and health status that evaluates health, socioeconomic, pharmaceutical, and diet histories, anthropometric measures, physical examinations, and laboratory tests.
What is nutrition care process and its four steps with question asked in each step?
A standardized-model tool utilized by nutrition and dietetics professionals to improve the consistency and quality of
individualized care for patients/clients or groups The NCP consists of four interrelated steps, performed in this order:
- Nutrition assessment: What fundamental nutrition problems
are present?
* Patient data are collected: biometrics, health history,
anthropometric data, dietary patterns - Nutrition Diagnosis
Nutrition diagnosis: What specific medical conditions have these problems caused?
● A PES statement (or Nutrition Diagnosis Statement) is a structured sentence that describes the specific nutrition problem that you (the dietitian) is responsible for treating and working toward resolving, the cause/s of the problem and the evidence that this problem exists.
* A PES statement has three elements: problem (P), etiology (E),
and signs and symptoms (S). - Nutrition Interventions
What changes should be put in place to address these issues, and how exactly will these changes be enacted?
● Considers the client’s barriers to
change - what is inhibiting him making
these changes? Finances, accessibility,
family support, etc. - Monitoring and Evaluation
How is the individual responding to the intervention(s)? Do
any intervention(s) need to be adjusted in some way?
What is anthropometric data?
- A non-invasive process
for determining body
fat mass - Incorporates a number
of human body dimensions
What is biochemical data?
- The concentration of chemical substances in tissue and body fluids
- The appearance of different metabolites
- Literature supports the statement that laboratory markers are not reliable as a standalone assessment tools
● Ranges for disease are broader than ranges for imbalances, toxicities, inflammation, oxidative stress, etc.
What is clinical data?
Information such as medical diagnosis, recent hospital admissions,
medications, changes in intake of food and fluids, food supply and preparation ability, weight changes
What is dietary data?
Population statistics, dietary surveys, food diaries, 24-hour recall, food-frequency questionnaires, food-habit questionnaires
