At the scene not requiring a PCR or NYFIRS report the company officer has the option of completing a
Unusual occurrence report
If a CFRD company applies and turns on the AED during cardiac arrest resuscitation attempts the original PCR shall be
Immediately faxed to Oma on return to quarters
When applying a AED a copy of the original PCR shall be
Turned into the CFRD depot along with AED module upon completion of assignment
Note the serial number of the module shall be. Documented on PCR
CFRD depot upon receiving a AED module used From a company will
Give them blank module Immediately Forward the module and copy of PCR directly to Oma
If making a correction on the PCR members shall
Place a single line through the word to be omitted or corrected and write the accurate info above the be corrected
Place initials of correcting member above the single line
If unable to obtain a persons name to put on PCR you should
Write UNKOWN and description of gender
If members name is circled at the bottom of PCR this means
The member that filled out PCR
All members of the CFRD engine or company should be documented on the bottom of the PCR
No only CFRD certified members who responded to and provide patient care
If chauffeur does not provide patient care do not place on bottom
