In Phase 1 of cardiopulmonary rehab, what MET level will activities be?
Met Level 1-2
- bed mobility, static standing
- transfers to bed/chair/bedside commode
- bed bath, feeding, grooming while sitting at sink
- AROM/warm up exercises
- Wheelchair mobility or ambulation around room
In Phase 2 of cardiopulmonary rehab, what MET level will activities be?
Activities will begin at MET levels 4-5 and gradually increase as person’s tolerance improves
**note patients are usually discharged to phase 2 when they can carry out activities at MET level 3.5
What is the primary treatment focus in Phase 3 of cardiopulmonary rehab?
OT provided as necessary for IADLs, leisure pursuits, and work.
Pt recovery focused on weight training to maintain strength and cardiovascular training. Groups may be integrated in this phase.
What are the primary OT interventions for Phase 1 of cardiopulmonary rehab?
- edema management secondary to lymphatic dysfunction
- exercise/stretching at low to mod intensity
(strenuous activity is contraindicated- will exacerbate lymphedema) - occupation based interventions
- education (skin care, don/doff compression garments, environmental mods, energy conservation, work simplification)
What are the breathing recommendations for cardiopulmonary patients?
- Diaphragmatic breathing which decreases energy required
- Pursed lip breathing which decreased rate of breathing and expels trapped air
- should be done during ALL activities/exercises
What is the focus of treatment in Phase 2 of cardiopulmonary rehab?
- Building up activity tolerance
- Educating patient on importance of continue exercise
- Improve ability to carry out IADL and community activities, especially work activities
What is the setting and time frame for phase 1 of cardiopulmonary rehab?
Acute care/inpatient, LOS usually 3-14 days depending on situation
What pressure level for a compression garment is contraindicated for cardiopulmonary patients
anything greater than 45mm/ Hg
What are absolute contraindications for cardiac patients?
- acute MI within 2 days
- unstable angina not previously stabilized
- uncontrolled arrhythmias causing symptoms
- acute PE
- acute myocarditis
- acute aortic dissection
