Pregestational Diabetes: risks to baby
- congenital defects to CNS and heart
- macrosomia
- hypoglycemia
Pre-clampsia onset

- early: before 34 wks
- late: after 34 weeks
- new onset: postpartum
Preclampsia phase 1 vs 2
Phase 1- early placenta; growth of narrow vessels with bad placental perfusion
Phase 2- Mom develops s/sx’s. Inflammatory response, affecting brain, liver, kidneys, vascular system
Preeclampsia Risks to Mom
- cerebral & pulmonary edema
- hemorrhage/stroke
- CHF
- abruptio placenta
Preeclampsia Risks to Baby
- IUGR (intrauterine growth restriction)
- premature birth
- intolerance to labor (ie stillbirth)
- risk for diseases later in life
Preeclampsia assessment (severe)
- BP>160/110 more than 1 reading
- visual problems
- proteinuria
- new SOB/pulmonary edema
- abnormal kidney/liver tests (RUQ pain from liver)
- thrombocytopenia
- HELLP syndrome
Preeclampsia Care
Goal and nursing care
- Goal: control BP, prevent seizures and bleeding
- Care:
-assess CNS, vision changes, clonus, DTR, RUQ pain, edema, lung sounds, urinalysis
-maintain bed rest and lateral recumbent position
Preeclampsia meds
3 (and their MOA)
- mag sulfate (blocks calcium, vasodilates)
- antihypertensies (vasodilate)
- anticonvulsants (binds to CNS receptors to stabalize cells)
Eeclampsia care
- pt safety
- airway
- lateral position
- monitor length
- fetal monitoring
HELLP Syndrome
- Hemolysis (destruction of RBC)
- Elevated Liver enzymes
- Low Platelets
Infection risk to fetus
- PTD
- IUGR
- LBW
- Sepsis
- Neurological damage
