Never Events
The CMS names select SREs as Never Events- adverse events that should never occur in a health care setting. Hospitals are denied higher payment for any hospital acquired condition resulting from or complicated by the occurrence of certain never events
Restraints
- physician order required
- order and renewal is limited to 4 hr for adult, 2 hr for 9-17 and 1 hr for children< 9
- Must meet one of the objective: reduce risk of patient injury from falls, prevent interruption of IV/NG tube/foley, prevent patients from removing life support equip, and reduce risk of injury to others by the patient.
What is mnemonic RACE in case of fire
R- rescue and remove all patients in immediate danger
A- activate the alarm.
C- confine the fire by closing doors and windows and turning off ox and electrical equip
E- Extinguish the fire.
Seizure
hyperexcitation and disorderly discharge of neurons in the brain leading to sudden, violent, involuntary series of muscle contractions that is paroxysmal and episodic, causing loss of consciousness, falling, tonicity, and clonicity.
Aura
before a convulsive episode a few patients report an aura which serves as a warning or sense that a seizure is about to occur. Its a bright light, smell, or taste.
What are risks for adolescent safety
Adolescents are at risk for injury from automobile accidents, suicide, and substance abuse
What are risks for adult safety
threats to an adult safety are frequently associated with lifestyle habits
Postictal Phase
follows a seizure, amnesia or confusion occurs and person falls into deep sleep
