Tricyclic antidepressants (TCA)
amitriptyline
imipramine
nortiptyline
trimipramine
- block reuptake of norepinepherine and serotonin
- 10-14 days to work, max effects in 4-8 weeks
Complications of TCA
amitriptyline, imipramine, nortriptyline, trimipramine
orthostatic hypotension
antichoinergic effects
sedation
toxicity
decreased seizure threshold
diaphoresis
increased appetite
contraindications of TCA
amitriptyline, nortrityline, imipramine, trimipramine
- category C
- seizure disorder
- caution with CAD, DM, liver/kidney/respiratory issues, urinary retention
- can increase suicide risk
Selective serotonin reuptake inhibitor (SSRI)
fluoxetine
paroxetine
sertraline
citalopram
escitalopram
-block reuptake of serotonin
Complications of SSRI
fuloxetine, paroxetine, sertraline, citalopram, excitalopram
sexual dysfunction
CNS stimulation (insomnia, agitation, anxiety)
weight changes
serotonin syndrome
withdrawal syndrome
hyponatrmia
rash
GI bleeding
bruxism
sleepiness, faintness, lightheadedness
Contraindications of SSRI
fluoxetine, paroxetine, sertraline, citalopram, escitalopram
- category C
- oxetine’s increase birth defects
- MOAIs or TCAs
- caution with liver and/or renal dysfunction, cardiac disease, seizure, DM, ulcer, hx of GI bleeding
