CH 16 General Principles of Managing MSD's Flashcards
better to focus on communication than just speech. T or F?
Ture
What are 3 management goals for treating MSD’s?
restore lost function, promote use of residual function, reduce need for lost function
what are factors influencing management?
medical dx and prognosis, disability and societal limitation (is it something that needs to be worked on), environment (supportive people in environment), associated problems (ex: cognitive problems making it difficult to manage MSD’s), health care system (money)
approaches to management
medical intervention, prosthetic, behavioral management
specify medical intervention
meds, surgery
specify prosthetic management
palatal lift for VP closure, pacing board, pointing to first letter of word, DAF, voice amplifier, light pointers (to point to letter or pictures)
what is the goal of behavioral management?
to maximize communication by whatever means that will produce the most rapid, effective, natural results
what are two types of behavioral management?
speaker oriented approaches and communication oriented approaches
what are speaker oriented approaches?
goal is to improve speaker. oral motor exercises to improve strength and compensatory strategies like reduced rate and over articulation. increase physiological support
what are communication oriented approaches?
structure interaction to improve speaking strategies. “If youre not understood do ___ and ___”
guidelines for behavioral management
start as soon as possible, baseline data and change depending on progression or regression of disease, initial focus of treatment should be to increase physiologic support of subsystems (BEST to start bottom up- respiratory first, phonatory next, resonance and articulation, compensatory methods last bc requires conscious and effort
principles of motor learning for treatment
practice is essential so pt must speak, drill is necessary but brief periods of practice over time is better than longer less often, client ownership and clinician should fade instruction, give feedback, use speech tasks not nonspeech tasks, begin with most advanced skill that pt has demonstrated in assessment
