CrCl = 98 - 0.8 X (patients age in years - 20)

Weights to use for calculating CrCl
Underweight (TBW < IBW) : Use TBW
Normal weight (TBW = IBW) : Use IBW
Overweight or Obese:
If BMI is GREATER than 25: use IBW
If BMI is LESS than 25: use (AdjBW)

IBW = 50 kg + 2.3 kg for each inch over 5 ft
Ideal body weight for females (IBW)
IBW = 45.5 kg + 2.3 kg for each inch over 5 ft
