closing mechanics of C spine joints?
extension, SB and rot. to the same side as the pain
what are cloward’s signs
points of pain at different places around the scapula that corrolate to C3-T1 discs
pt profile: deep, dull ache, clowards signs,
discogenic pain
what happens to the pain pattern for discogenic pain if it is a posteriolateral disc problem?
spreads to upper arm pain as well as scapular
pt profile: age
disc herniation, tx with traction and mckenzie’s repeated extension exercises
pt profile: middle aged, deep ache, loss of cervical lordosis, agg: sustained flexion (reading), quick movements, end range movements, loooooong history of neck pain, maybe sec. to MVA? central and unilat. PAs hurt, limited ROM
disc degeneration in C spine–> hypermobility –>spondylolysis
tx with scap stab. and thoracic extension, deep neck flexion ex.
pt profile: local pain on ipsilat. side, SB is limited to both sides, stiff PAIVMS and PIVMS
facet lock, joint specific traction
pt profile: either young with hx of trauma or old with hx of degeneration, pain is worse distally in a dermatomal pattern, may have clowards signs, constant and or latent pain. Agg: any closing movement of the neck, arm movement, sustained flexion, wakes up at night, uncomfortable posture, limited ROM due to pain
acute nerve root, wait for severity to decrease then do joint mobs and traction
pt profile: pain in dermatomal pattern but more prox than distal, patchy, intermittent distribution of pain. agg: sustained flexion, closing movements, nagging pain but able to sleep at night, central PAs painful, pos. spurling’s test, x ray shows degen. changes
chronic nerve root
tx: joint mobs, traction
pt profile: >55 years old, hx of high contact sports, painful, restricted ROM, neck and arm pain, loss of hand dexterity
cervical stenosis
