CEREBRAL VASCULATURE tutorial Flashcards
two type of strokes
hemorrhagic stroke and ischemic stroke
what type fo stroke is it likely to be if someone has fallen off bike, hit left side of head theyr eknocked out for a bit but fine and after five days they have right limb weakness
haemmorhagic stroke particularly from subdural haemmorhage bc of delayed sympotms
is subdiral haemorrhage venous or arterial
venus
what might cause a severe headache in the case of a subdural haemorrhage?
gradual buildup of intracranial pressure
what may be causing drowsiness and speech problems in subdural haemorrhage?
reticular acitvating system (RAS) being compressed
in brain CT where is right and left
patients r/l is opposite to image bc we look at patient form feet to head like in anatomy
where in brain is there haematoma if motor sympotms on right
in left side motor system
what does a subdural haematoma look like in head CT
black blob in outer end
look at ct from insendi if u dont get it what may happen to ventricle on the side of the brian where theres a bleed
ventricle may become smaller bc its compressed by the bleed
what is the way to approach a head ct
look at symmetry - start by looking at mid line, is it straight?
how would an ACUTE bleed look on CT
a WHITE blob- look at ct its the white thing in middle
which part of brain do the symptoms of: drowsy, slurred speech, wobbly eye movements and loss of reflexes, ataxia ( eg clumbsy hands ) point to?
cerebellum
what type of brain bleed is it if its in the cerebellum?
intraparenchymal meaning in substance of brain and specifically intracerebellar
what other things other than the sympoms can point at an intracerebellar bleed
rapid onset, lack of history of head injury, patient history
what is the treatment for this spontaneous intracerebellar bleed?
manage the hypertension. So this doesn’t happen again.
Sometimes the surgeons might go in and try to remove blood.
(very unlikely in this particular case because it really is at the core of the cerebellum, just above the brain stem.= not very easy to approach for the neurosurgeons without causing more damage.)
this lady with the acute bleed had forgotten to take her antihypertensive meds. thats common. why? what does this mean for docs?
bc they dont associate the med with a specific sympotm- low motivation and actually may have side effects- even less motivation, docs need to make sure patients are taking them, let them know risks ect.
