What is cerebral palsy?
A
- Most common motor disorder in kids; motor and posture disorder
- Developmental brain injury
- Non-progressive (brain pathology does not get worse)

Ways to Classify Cerebral Palsy
A
- Pattern and distribution of motor problems
- Whether inc or dec tone
- Type of movements (ataxia, dyskinesia, mixed)
- Gross Motor Function Classification (1 most function - 5 least)

What is the most common type of CP?
A
Pre -term = spastic diplegia (both legs)
Term = spastic hemiparesis

8 Diff Poss Pathogenic Mechanisms that Cause CP
A
- Periventricular leukomalacia - injury that damages oligodendrocytes around lateral ventricles
- Hypoxia at birth (watershed areas affected most - located in peri-ventricular area in infants)
- Congenital brain malformations (genetic)
- Stroke in peri-natal period (may be due to placenta emboli)
- Maternal infection
- Post-natal infection (meningitis or sepsis)
- High bilirubin
- Brain trauma in infant

Pre-term v Term Babies w/ CP
A
- Preterm – periventricular leukomalacia -> spastic diplegia
- Term – arterial distributions -> hemiplegia, quadriplegia

Dx and Tx of CP
A
- Usually do not diagnose after only one visit; seen for not meeting milestones then rule out other things (+ sometimes motor deficits improve on own over time)
- 1- treat acute illness/acute underlying cause (neuroprotection)
- Ex) hypothermia to prevent further brain damage once born
- 2- treat to reduce spasticity (valium, baclofen, botulinum toxin injections)
- 3- therapy (changes as needs change across lifetime)
- Ex) physical, occupational, vision, speech, vocational
