Case 5 (stroke) Flashcards
1
Q
What are the treatment options for ischaemic stroke?
A
- tPA (thrombolysis within 4.5hrs)
- warfarin/heparin (anticoagulation)
- anti-platelet drugs (aspirin)
- endarectomy
- antihypertensives (if patient is hypertensive)
2
Q
What are the treatment options for haemorrhagic stroke?
A
- antihypertensives (reduces BP)
- anticonvulsants (reduce risk of seizure)
- mannitol (= osmotic diuretics, decrease ICP)
3
Q
What is the MOA of tissue plasminogen activator (tPA)?
A
- Inactivates thrombin and ADP
- tPA cleaves plasminogen into plasmin which initiates fibrinolysis = degrades thrombi (clots)
4
Q
What is an endarectomy?
A
Removes atheromatous plaques from internal carotid artery
5
Q
Where do stroke-causing thrombi usually form?
A
- bifurcation of common carotid into internal/external carotids
- aortic arch
6
Q
What do osmotic diuretics (e.g mannitol) do to treat haemorrhagic strokes?
A
Decrease intracranial pressure (ICP) in subarachnoid space
