Do you think I have a brain tumor?
A
I think it’s unlikely. To make absolutely sure, however, we will do a CT scan, which is a special imaging study of the brain.
That will help us see the structure of the brain and rule out any bleeding or tumor.

Specific Qs:
A
- Describe what happened.
- Loss of consciousness before, during, or after the fall.
- Duration of loss of consciousness.
- Palpitation before the fall.
- Sensing something unusual before losing consciousness (sounds, lights, smells etc).
- Spinning/lightheadedness.
- Shaking (seizures).
- Duration of shaking.
- Bit tongue.
- Lost control of the bladder.
- Speech difficulties.
- Confusion after regaining consciousness.

PEx:
A
- Head and neck exam ==> Inspection (head, mouth), carotid auscultation and palpation, thyroid exam.
- CV ==> P + A + Orthostatic VS.
- Chest ==> A.
- Extr ==> Palpated peripheral pulses.
- Neuro ==> Mental status, CNs (including fundoscopic exam), motor exam, DTRs, cerebellar, Romberg test, gait, sensory exam.

DDx:
A
- Convulsive syncope.
- Cardiac arrhythmia.
- Seizure.
- Vasovagal syncope.
- Drug-induced orthostatic hypotension.
- Aortic stenosis.

Workup:
A
- CBC.
- Electrolytes.
- ECG.
- CT head or MRI brain.
- EEG.
- Echocardiography.
- CXR.
- Prolactin (measured 30-60min post-seizure).
