Care Plans and Monitoring Problems Flashcards
The PPCP includes
assessing drug-related needs and problems
creating a care plan
follow-up to see if positive outcomes achieved
Before Care Planning
collect info from patient and EMR
understand the patient’s needs and goals
assess drug therapy regimen
Goals of care planning
establish therapeutic goals
determine interventions to resolve all existing drug therapy problems, achieve goals of therapy, and prevent future drug therapy problems
maintain active patient involvement
facilitate coordination with other care team members
Care plans and monitoring
incorporate evidence-based recommendations
documentation of problems, goals, and interventions
systematic, ongoing process of planning, action and documentation
dynamic, goal driven, specific to patient
Parts of a Care plan
medical condition
drug therapy problems
goals of therapy
interventions
follow up plan
Types of interventions
pharmacotherapy
non-pharmacological
lifestyle modifications
patient education
referrals to other health care providers
In care planning you should be specific with
recommendations and rationale
dosing and frequency
patient and drug therapy they are on
Using DI skills helps incorporate
evidence based treatment recommendations
Monitoring includes both
drug safety and efficacy of the drug and disease state
A care plan is incomplete without a ?
follow up plan
What happens at a follow-up visit?
evaluate the positive and negative impact of the care plan
compare current status to patient specific goals
identify new drug therapy problems
address new problems and or adjust previous therapies as needed
Resolved
Therapeutic goals achieved for the acute condition; discontinue therapy
Stable
Therapeutic goals achieved; continue the same therapy for chronic disease management
Improved
Progress is being made in achieving goals; continue the same therapy because more time is required to assess the full benefit of therapy
Partial improvement
Progress is being made, but minor adjustments in therapy are required to fully achieve the therapeutic goals before the next assessment
Unimproved
Little or no progress has been made, but continue the same therapy to allow additional time for benefit to be observed
Worsened
A decline in health is observed despite an adequate duration using the optimal drug; modify drug therapy (eg, increase the dose of the current medication, add a second agent with additive or synergistic effects)
Failure
Therapeutic goals have not been achieved despite an adequate dose and duration of therapy; discontinue current medication(s) and start new therapy
Expired
The patient died while receiving drug therapy; document possible contributing factors, especially if they may be drug related
