Normalisation is the process of making it possible for an individual with a disability to lives as normally as possible in society. Normalisation is about changing society to fit the individual as opposed to changing the individual to fit society.

Describe the Assessment stage of Care Planning

A

Assessments should be carried out so that service users and their carers can;
Gain a better understanding of their situation
Identify the options that are available
Set aims and goals to be achieved over the course of the care plan
Understand the basis on which decisions are reached

Assessments should be coordinated and integrated all agencies relevant to the user.
Assessments should be shared among professionals, with the informed consent of clients that assessments are not repeated.
At every stage of the assessment the individual and carers views must be included.
The first assessment will be done by the professionals who have first contact. Other specialist assessments may be done at a later stage.

A

The information gained from the Assessment stage should be used to produce an individual care plan, including;
Identifying the individuals care needs
Setting goals or objectives that care workers should aim to achieve
Identify the type of care the person requires and the way it will be provided
Care interventions should be carefully selected to meet the individuals specific care needs.

A

Parent/carer advocacy is where someone’s parent or carer will be speaking for the patient, normally in this case it is for someone under the age of 18 or an adult with a head injury.
