  • Suspected cocaine toxicity with chest pain

A
  • Hypotension under 90 systolic
  • Hypotension under 110 for acute heart failure
  • Head trauma
  • Cerebral haemorrhage
  • Viagra and other related drugs taken within a 24 hour period
  • Unconscious patient
  • Known severe aortic or mitral stenosis

A

Cardiac Chest Pain:
INITIAL: 400-800mcg spray
REPEAT DOSE: 400mcg after 5-10 minutes
MAX DOSE: No limit

Acute Heart Failure:
INITIAL: 400-800mcg
REPEAT DOSE: 400mcg after 5-10 minutes
MAX DOSE: 6 sprays (2.4mg)

  • Active bleeding

  • Patients at increased risk of bleeding (see JRCALC for list)

Indications of Aspirin

A
  • Adults with clinical or ECG evidence suggestive of an MI or ischaemia
  • Suspected TIA. Symptoms must be fully resolved

  • Known allergy
  • Under 16
  • Active GI bleed
  • Clotting disorders
  • Sever hepatic failure with jaundice

Indications of Atropine

A
  • Symptomatic bradycardia
  • Absolute bradycardia <40
  • Systolic bp less than expected in paeds
  • Paroxysmal ventricular arrythmias requiring suppression
  • Inadequate perfusion causing confusion
  • Bradycardia following ROSC

Contra-Indications of Atropine

A
  • Do not give to treat bradycardia in suspected hypothermia
  • Pts with cardiac transplants; their hearts will not respond to vagal blocking by atropine which may cause high AV block or sinus arrest

A
  • Reverses effects of vagal overdrive
  • Increases heart rate by blocking vagal activity in sinus bradycardia, second or third degree heart block
  • Enhances AV conduction
