The Coronary Vessels; What each Supplies Blood To

A

Aortic Ostia - supplies cardiac muscle blood
Main Vessels:
- Right Coronary Artery
- Left Coronary Artery which splits into left anterior descending artery and circumflex artery

LCA - supplies left ventricle and left atrium
LAD - supplies blood to the front and left side of heart, septum
Circumflex - supplies the outer side and back of heart
RCA - supplies blood to righ ventricle, left atrium, septum and the electrical nodes

Arteries - carry blood away from the heart into arterioles Arterioles - Branches of arteries to control flow to capillaries Veins - carry deoxygenated blood from capillaries back to heart Capillaries - carry blood straight to cells, to diffuse with cell fluid to exchange materials with tissues

Pathway of Electricity in the Heart Relating to Muscle Movement

A
  1. The Sinoatrial node (SAN) located in the right atrium stimulate releases an electrical impulse. This causes the contraction of the atria
  2. The wave of excitation travels to the atrioventricular node (AVN) located in the bundle of his, in the septum
  3. The AVN causes electricity then travels along the bundle of his (septum) to the Purkinje tissue at the bottom and upwards across the ventricles causing the ventricles to contract

  • Acute heart failure with ischaemia or uncontrolled hypertension
  • Suspected cocaine toxicity with chest pain

  • Hypotension under 90 systolic
  • Hypotension under 110 for acute heart failure
  • Head trauma
  • Cerebral haemorrhage
  • Viagra and other related drugs taken within a 24 hour period

  • Known severe aortic or mitral stenosis

Dosages of GTN

A

Cardiac Chest Pain:
INITIAL: 400-800mcg spray
REPEAT DOSE: 400mcg after 5-10 minutes
MAX DOSE: No limit

Acute Heart Failure:
INITIAL: 400-800mcg
REPEAT DOSE: 400mcg after 5-10 minutes
MAX DOSE: 6 sprays (2.4mg)

A
  • Potent vasodilator resulting in the dilation of coronary arteries/relief of coronary spasm
  • Dilation of systemic veins resulting in lower pre-load
  • Reduced BP

  • Headache
  • Dizziness
  • Hypotension

Indications of Furosemide

A

Consider IV furosemide for pulmonary oedema and/or respiratory distress due to acute heart failure

A

20 mg in 2ml ampoule

50mg in 5ml ampoule

Actions of Furosemide

A

A diuretic with a rapid onset of around 30 minutes. Has a short duration of effect.

A
  • Reduced GCS with liver cirrhosis
  • Cardiogenic shock
  • Severe renal failure with anuria
  • Less than 18 years old
