Q

Name some structural differences between arteries and veins

A

Arteries transport blood away from the heart
Veins carry blood towards the heart.

Arteries = high pressure
Veins = Low pressure

Both have three layers

Arteries = narrow lumen
Veins = wider lumen

Arteries have a thicker tunica media (more elastic tissue)

Veins have valves to control blood flow

Q

Initial investigations for hypertension?

A

Rule out secondary causes of HTN

Calculate Cardiovascular Disease Risk (QRISK)

Urinalysis: Test for the presence of proteinuria and haematuria and urine sample for Albumin:Creatinine ratio

Bloods: FBC, HbA1C, Haematocrit, eGFR, Lipid profile, TFTs, U&Es

Fundoscopy (hypertensive retinopathy)

Arrange for a 12-lead ECG

Q

What is an ACE inhibitor? Explain it’s MOA

A

Works by blocking Angiotensin Converting Enzyme (ACE), which normally contributed to Angiotensin II production.

Angiotensin II is a vasoconstrictor and triggers water retention which both contribute to increasing blood pressure.

Q

When treating hypertension, what is the target blood pressure measurement to maintain if the patient is UNDER 80 years old, according to NICE Guidance?

A

Below 140/90 mmHg

17
Q

When treating hypertension, what is the target blood pressure measurement to maintain if the patient is OVER 80 years old, according to NICE Guidance?

A

Below 150/90 mmHg

18
Q

Why is blood pressure measured on both arms at the first visit?

A

To find three major health conditions:

Coarctation of the aorta

Subclavian artery stenosis

Thoracic aortic dissection
