What are the signs of heart failure?
- High right pressures: increased JVP, hepatomegaly, ascites, edema
- Pulmonary htn: parasternal lift, palpable and loud P2
- High left pressures: tachypnea,
Inspiratory crackles, S3, hypoxia - Low cardiac output: decreased LOC, cool extremities, low urine
Which drugs prolong survival in heart failure?
- Ace inhibitors
- Angiotensin receptor blockers
- Beta blockers
- Aldosterone inhibitors (in NYHA 3-4)
Which drugs only improve symptoms in heart failure?
- Diuretics
- Nitrates
- Digoxin
How is heart failure with preserved ejection fraction managed?
No medical tx for survival benefit.
What drugs can control atrial fibrillation?
- Propafenone: combo therapy
- Sotalol: never use with long QT
- Amiodarone
How does heart failure affect coronary perfusion?
Heart failure increases left ventricular end diastolic pressure, which decreases the gap between aortic pressure and LV pressure, which leaves less of a gradient, therefore decreases coronary perfusion pressure
What tests might show heart failure?
Chest X-ray: cardiomegaly, bat wing opacity around hilum, Kerley B lines
NT-proBNP
Echo: good for diagnosing mechanism of heart failure
What can cause left sided heart failure?
- Aortic coarctation/ severe HTN
- Aortic valve stenosis or insufficiency
- Left ventricular systolic or diastolic dysfunction
- Mitral valve regurgitation or prolapse
What can cause isolated right heart failure?
- Lung disease with pulmonary HTN
- Pulmonic valve disease
- RV dysfunction
- Tricuspid valve disease
What is the NYHA functional classification?
- No SX within normal range of activities
- SX with ordinary activities
- SX with less than ordinary activity
- SX at rest
What maladaptive responses does the body have to decreased cardiac output?
- Increased sodium and water retention: increases SV initially but over time causes congestion and progressive LV remodelling
- Increased SVR: increases your blood pressure (good) at first, but over time decreases stroke volume and co
How is acute decompensated heart failure treated?
- ID cause
- Basic ABCs
- Nitrates and diuretics
- Morphine
- Beta blockers will make people feel worse!
How can refractory heart failure be treated?
- Optimize medical tx
- Biventricular pacing
- Left ventricular assist device
- Transplant
