What is the most common cause for ACS?
Rupture of atherosclerotic plaque
Risk factors for CVD?
Unmodifiable; increasing age, male gender, family history
Modifiable; smoking, diabetes mellitus, hypertension, hypercholestrolaemia, obesity
Pathophysiology of ischaemic heart disease?
Endothelial dysfunction
Resulting in pro-inflammatory, pro-oxidant, reduced NO availability
Infiltration of subendothelial space by LDL
Monocytes migrate into subendothelial space and differentiate to macrophages which oxidise LDL and become foam cells
Smooth muscle proliferates and migrates to tunica intima and forms a fibrous cap over fatty plaque
This narrows lumen and results in reduced blood flow to the myocardium
