MI
PCI / CABG Thromoblysis - alteplase / streptokinase within 12 hours Beta blocker - bisoprolol ace-i (rampiril) Clopidogrel GpIIb/IIIa - abciximab
After = daily aspirin, bb,acei, simvastatin
Angina
Statins - simvastatin Aspirin and clopidogrel Beta blockers - bisoprolol Nitrate - gtn Ccb - amlodipine Long acting nitrate - isosobide nitrate
Peripheral artery disease
Treat as if angina + percutanenous transluminal angioplasty within 4-6 hours
Cardiac tamponade
Pericardiocentensis
Pericardial effusion
Treat underlying
If reoccurent make fenestrations
Infective endocarditis
Blind - amoxicillin and gentamicin
Staph - flucloxacillin and gentamicin
Strep - benzylpenicillin and gentamicin
Atrioventricular nodal reentrant tachycardia / not nodal
Catheter ablation
Vagalmanoeuvre or carotid sinus massage
Cardioversion
DVT / PE
LMWH - ENOXAPARIN
Warfarin
Stockings
Pericarditis
NSAIDS
Colchicine
Aspirin
Bed rest
Bradycardia
Stop drugs that may contribute
Symptomatic - atropine (mAChR antag)
Atrial flutter
Cardioversion BB - BISOPROLOL Ccb - verapamil Digoxin - inhibit na/k atpase Amiodarone
Atrial fibrillation
Rate control - bb (bisoprolol) ccb (verapamil) if these fails digoxin
Rhythm control - amiodarone / cardioversion
Anticoagulants (warfarin)
Chronic heart failure
1) lifestyle modification
2) treat underlying cause
3) avoid nsaids and verapamil
4) loop diuretics - Furosemide
5) bb - bisoprolol
6) spironolactone
7) digoxin
8) vasodilators
