How many phases in CR provision?
Phase I
Phase II
Phase III
Phase IV
Phase I =
inpatient program
begins soon after a cardiac event (such as a heart attack, angioplasty, or bypass surgery)
finishes when the patient is ready to go home from the hospital
emphasis is on low-level exercise and education for the patient and family
Phase II =
outpatient hospital-based program
begins approximately 2 weeks after discharge from the hospital
dietitians, social workers, pharmacists, physicians, and others may also be involves in care
emphasis on monitored exercise and continued education on exercise and lifestyle management
Phase III =
ongoing community-based exercise and eduction program supervised by cardiac nurses and specialists
spouses and significant others may also participate in this maintenance program with a referral from a physician
Phase IV =
continuation of phase II but without supervision
patients continue to apply what they have learned during the preceding phases
