A

Vfib/pulseless vtach- 300 mg in 20 ml NS IV, repeat w 150 mg

Vt/wct- 150 mg in 50 ml NS over 10 min

Maintenance infusion- 1 mg/min over 6 hr (150 mg in 250 ml)
