EPI dose cardiac arrest
1mg repeat every 3-5mins
Epi dose pressor
5-20mcg every minutes
EPI indications
Cardiac arrest, pressor, symptomatic bradycardia refractory to atropine.
Dopamine indications
significant hypotension no from hypovolemia, cardiogenic shock, symptomatic bradycardia refractory to atropine
Dopamine Dose
2-20 mcg/kg/min titrate to effect
Lidocaine indications
V-tach and V-fib refractory to Amiodarone
Lidocaine dose
1-1.5 mg/kg repeat doses of 0.5-0.75 mg/kg every 5-10 minutes max of 3mg/kg
Adenosine indication
SVT, Stable VT, and other tachyarrhythmias refractory to vagal. NOT TORSADES NOT AFIB
Adenosine Dosage
6mg IV push rapid flush
if unconverted
Immediate 12mg IV push rapid flush
Diltiazem/Cardizem indications
Stable narrow tachyarrhythmias/recurrent SVT refractory to adenosine and vagal. Afib/aflutter
Dosage of Diltiazem
1st dose 0.25mg/kg over two minutes
wait 15 minutes
2nd dose 0.35mg/kg
may be followed by a 5-15mg/hr infusion
Indications for Amiodarone
Afib, SVT, vtac, vfib
Cardiac arrest dose of Amiodarone
For VFIB an pVTAC
First 300mg
4 minutes
Second 150mg
Dosage of Amiodarone for narrow complex tachycardias
150mg over 10 minutes
can be followed by 1mg/min infusion over 6 hours
Indications for atropine
Stable bradyarrhythmias
Dosage for atropine
0.5mg-1mg doses repeat 3-5 minutes max 3mg
Organophosphate poisoning 2-5mg
Indications for Mag Sulphate
Torsades
Dosage for Mag Sulphate
1-2 mg over 15 minutes
Indications for Sodium Bicarb
Acidosis, TCA overdose
Dosage for Sodium Bicarb
1mEq/kg
Furosemide Indications
CHF Pulmonary Edema
Dose of Furosemide
40-80mg
Indications for Calcium Chloride
Hyperkalemia, hypocalcemia, calcium channel blocker toxicity (nifedipine, verapamil, diltiazem)
Dosage for calcium chloride
2-4mg/kg every 10 minutes.
