Adenosine
SVT
1st: 6mg rapid IVP followed by a 20cc flush
2nd: 12mg rapid IVP followed by a 20cc flush
Amiodarone

1st: 300mg IVP
2nd: 150mg IVP
Aspirin

Atropine

0.5mg (max: 3mg)

1mg 1:10,000 IVP every 3-5min.

2-20 mcg/kg/min in 250mL D5W over 10 minutes
Diltiazem

1st: 0.25mg/kg
2nd: 0.35mg/kg
Lidocaine
Cardiac Arrest
1-1.5mg/kg IV
4mg/mL 1-4mg per minute
Calcium Chloride
INDICATIONS- Hypocalcemia, hyperK+, hypermagnesemia, beta blocker toxicity, calcium channel blocker toxicity.
CONTRAINDICATIONS- Hypercalcemia, V-Fib, digitalis toxicity.
DOSAGES- Calcium channel blocker OD & hyperK+: 500-1,000mg (5-10mL of 10% solution) IV push. May repeat as needed.
Dobutamine Hydrochloride (Dobutrex)
INDICATIONS- CHF, cardiogenic shock, L ventricular dysfunction, often used in conjunction w/ other drugs.
CONTRAINDICATIONS- Tachydysrhythmias, severe hypotension, idiopathic hypertrophic subaoritc stenosis (IHSS), suspected or known poison/drug-induced shock.
DOSAGES- IV infusion: @ 2-20 mcg/kg/min titrated to desired effect. Max dose 40 mcg/kg/min.
Dopamine Hydrochloride (Intropin)
INDICATIONS- Cardiogenic & septic shock, hypotensive w/ low cardiac output, distributive shock, 2nd line drug for symptomatic bradycardia.
CONTRAINDICATIONS- Hypovolemic shock, phenchromocytoma, tachydysrhythmias, V. Fib, V. Tach
