Cardiac arrhythmias Flashcards
Narrow QRS
Supraventricular origin
Wide QRS
Ventricular origin
Supraventricular tachycardias
AF, Atrial flutter, ectopic atrial bradycardia
Bradycardia
Sinus bradycardia and sinus pauses
Ventricular arrhythmias
Ventricular ectopics/premature ventricular complexes, ventricular fibrillation, asystole
Symptoms of arrhythmias
Palpitations, dyspnoea, presyncope, syncope, shock, sudden cardiac death, angina, hf, anxiety
You suspect arrythmias
- 12 lead ECG
- Blood
- CXR
- Echo
- Stress ECG
- 24hr holter monitoring
- Event recorder
- EP study - lol you trigger it to test it how weird
Treating sinus bradycardia
Atropine if acute
Pacing
Treating sinus tachycardia
Treat underlying cause
Beta blockers
Treating atrial ectopic beats
Nothin lol but sometimes b blockers
Management of acute supraventricular tachycardia
Increase vagal tone w/ valsalva or carotid massage
ADENOSINE OR VERAPAMIL
Management of chronic supraventricular tachycardia
young, symptomatic patient - EP study and RCFA
Beta blockers and anti-arrhythmics
RCFA
Radiofrequency catheter ablation - selective cautery of cardiac tissue to prevent tachycardia
Causes of heart block
Ageing, MI, Myocarditis, Infiltrative disease, Drugs, Aortic valve disease, post-aortic valve surgery, genetic
Treatment of second degree MII block
Pacemaker
