Cardiac Flashcards
For which rhythms is cardioversion indicated?
A fib, A flutter and supraventrucular tachycardia
Why should Metoprolol not be given to a PT with a 2nd degree AV block?
Metoprolol would further slow down the heart rate and worsen the block.
What is the effect of Ondansetron on the heart?
Prolongs QT (cardiac output).
Characteristics of a junctional rhythm?
1) Narrow QRS
2) Slow ( < 60 bpm); when faster, is referred to as a junction all escape rhythm.
3) P Wave is hidden in QRS or can be inverted
Hypertension is a risk factor for developing which cardiac rhythm?
A Flutter
Which lab value is of concern in a PT with Rhabdomyolysis?
Hyperkalemia
Which medication should be given to a PT with Rhabdomyolysis and a serum potassium of 7.0? Why?
Calcium Gluconate - IV Calcium Gluconate will prevent ventricular dysrhythmias by stabilizing the action potentials of the myocardial cells.
Magnesium Sulfate is given for which cardiac rhythm?
Torsades de Pointes
Briefly describe the use Central Venous Pressure monitoring.
CVP is a measure of BP in the right venae cavae near the right atrium. It reflects preload and changes in response to infusions of IV fluids. Normal CVP is 3 to 8 mmHg.
How do you treat Cardiogenic shock?
Fluids and plasma
Meds to increase hearts pumping abilities - ie digoxin
Also give meds such as Levophed or dopamine
What is Cardiac Index?
It’s a measure of Cardiac Output/Body Surface Area. It relates heart performance to the size of the individual. Normal = 2.6 to 4.2 L/min.
CI below 2.6 May indicate cardiogenic shock.
