CA C spine rules
if 1 high risk factor - radiograph
if 2 low risk factors and can’t rotate to 45 deg bilaterally - radiograph
Cervical myelopathy - clinical presentation
(+) babinski, (+) hoffman, gait deviation, >45 yo, positive inverted supinator sign
Cervical radiculopathy - CPR
(+) traction test, (+) ULTT median nerve, (+) Spurlings, limited movement to one side (<60 deg rotation)
Cervical Manip
NDI <5 hrs/day, feels better to move neck, extending neck does not worsen sx, spondylosis w/o radiculopathy
Cervical Traction
peripheralization with lower C-spine mobility testing, (+) shoulder abduction test (hand on head relieves sx), >55 yo, (+) median n test, (+) neck distraction test
Thoracic Manip for C-spine pain
sx <30 deg
How do you assess the C1-2 joint
in supine, bring the pt to max flexion and passively rotate (should be 45 deg bilat)
Neck flexor muscle endurance test findings
no neck pain: avg of 39 sec
with neck pain: avg of 24 sec
