C spine Flashcards
ROM evaluation - upper C spine
extension/protraction - hand on top of head to pull backwards, other hand around jaw to pull up.
flexion/retraction - hand in U at chin pushing AP, stabilise at occiput.
ROM evaluation - upper c spine quadrant
standing on side where theyre rotating to
protraction, hand on their chin and occiput to bring head in flexion towards me, add rotation towards same side to really close it down
ROM evaluation - lower C spine quadrant
retraction, lat rotation, lateral flexion
Palpation
supine:
- occiput base
- mastoid process
- C2 SP
- suboccipital muscles
- C7 SP
- paraspinal muscles
- articular pillars C2-7 aka Z joints
- sternocleidomastoid muscle - get them to protract to find it
- trapezius muscle
- temperomandibular joint
- thyroid cartilage
PAVIMs - mid/lower C spine
Central PA mob/glide - prone
Unilateral PA - supine
Transverse - from one side, 2 thumbs, push to other side on SP TO CHECK
Unilateral AP - prone
PAVIMs - upper C spine
Unilateral PA C2 - prone
Central PA on C0-1
Ligament testing - upper C spine (AA joint)
Sharp-purser test -(Co-1)
feel for C2 and stbailise aorund it in C grip.
apply slight flexion, do PA glide through other hand on forehead.
Ligament testing - C spine alar lig.
(C1-2)
stabilise C2, sight flxn.
rotation with other head on top of head.
lots of mvmt = damaged alar lig.
Traction
patient supine, me lunging at head.
hands overlapped under nuque at C2. apply cranial glide with very slight flexion by lunging backwards to not pull with arms.
for radicular pain
PPVIMs
for mobility issues
SNAGS C1/2, C2-7 rotation
down from mastoid process to TP with thumb to feel and other over to push.
sustain glide at upward 45˚ during rotation.
take baseline, treat (4-5sets) with overpressure and retest (PILL).
“better, same, worse pain”
SNAGS C1-7 flexion/extension
edge of one thumb on SP, other on top to push.
flexion:
- lift joint up, then move into flexion while holding pressure.
- overpressure at EOR
extension:
- same thumb position
- push then go up into extension , while holding pressure.
- overpressure
