what are some early complications of burns?
infection
hypovolaemia
metabolic disturbance - increased K, myoglobin, Hb which leads to AKI
compartment syndrome
peptic ulcers - curlings ulcers
pulmonary - laryngeal oedema, CO poisoning, ARDS
renal and hepatic failure
what are some intermediate complications of burns?
VTE
pressure sores
what are some late complications of burns?
scarring
contractures
psychological problems
what is the wallace rule?
rules of 9%
- percentage of body area burnt
state the wallace rule
head and neck 9% each arm 9% torso 18% back 18% each leg 18% perineum 1%
what is the management of burns ?
ABCDE
secure airway
manage fluid loss
prevent infection
how should respiratory burns be managed?
flexible laryngoscopy to identify damage
early intubation
dexamethasone
how can respiratory burns be identified ?
soot in oral or nasal cavities
burnt nasal hairs
hoarse voice, stridor
what are three signs of CO poisoning?
headache
confusion
cherry red appearance
what fluids should be given for burn patients ?
2L of warmed Hartmanns immediately
what is the treatment for burns ?
analgesia - morphine
dress partial thickness burns
- biological, synthetic, cream
full thickness burns
- debridement
- split thickness skin grafts
anti tetanus toxoid
consider prophylaxis Abx (anti pseudomonnal)
