What is the first thing we should be doing when a pt is in a thermal burn?
make sure that fire personnel is called and ensure that the pt is moved as quick as possible to a FRESH air zone!!!!
What are the life limb function threats listed?
airway burns
asphyxia
CO/cyanide poisoning
shock
What are other important things to note when assessing a burn?
- source of burn
- wether it occurred in an enclosed space
- if the pt was unconcious or lost LOC during the exposure to the fire/fumes/smoke
If the clothing is stuck to the skin, what should we do??
remove clothing and cut around the skin that is stuck to clothes
What is included in the the secondary survey?
severity: area burned, burn depth (degree), and % of body surface area burned
distal neurovascular status
signs for smoke inhalation and upper airway injuries
if burns involve the eye, assess as per the visual disturbance standard
For 2nd degree burns <15 %, what do we do?
dress with moist sterile dressing and dry sheet or blanket, dress each digit individually
For 2nd degree burns >=15%, what do we do?
dress with DRY sterile dressing, dress each digit individually
For 1st degree burns, what do we do?
dress with moist sterile dressings and then cover with dry sheet or blanket
Limit cooling to how long to prevent WHAT condition?
<30 mins
to prevent hypothermia!!!!!
If shivering starts or hypotension, what should we do?
discontinue the cooling efforts
What are potential expected problems of burns? (thermal)
airway obstruction
if airway burns,
- bronchospasm
- orolingual/laryngeal spasm
respiratory distress/arrest and
agitation or combativeness
