A

CAST: Clavulanic Acid, Sulbactam, Tazobactam. Added to Pcn to protect it from destruction by beta lactamases

16
Q

Name the carbapenems. What is special about one of them?

A

Imipenem & Meropenem. Must give cilastatin with imipenem to prevent inactivation of drug in renal tubules

17
Q

What do the carbapenems treat?

A

Gram+ cocci, Gram- rods, and anaerobes. However, limited in use due to significant AEs (seizures)

18
Q

Vancomycin treats?

A

Serious Gram+ infections: MRSA, enterococci, C. diff

19
Q

Name the AMGs

A

GNATS: Gentamicin, Neomycin, Amikacin, Tobramycin, Streptomycin

20
Q

AMGs treat?

A

Severe Gram- rod infections. Cannot treat anaerobes cuz need O2 for uptake. Neomycin for bowel surgery

21
Q

Doxycycline treats?

A

Borrelia burgdorferi, M. pneumoniae, & intracellular bugs (Rickettsia, Chlamydia)

22
Q

Demeclocycline treats?

A

SIADH.. it is an ADH antagonist & is rarely used as an antibiotic

23
Q

The macrolides (Azithromycin, Clarithromycin, Erythromycin) treat?

A

Atypical pneumonias (Mycoplasma, Chlamydia, Legionella), STDs (Chlamydia) and Gram+ cocci (Streptococcal infections for pts allergic to Pcn)

24
Q

Chloramphenicol treats?

A

Meningitis (H. influenza, Neisseria meningitidis, Strep pneumo). Used sparingly in US cuz of toxicities (anemia & gray baby syndrome)

25
Q

Clindamycin treats?

A

Anaerobic infections above diaphragm (aspiration pneumonia with Bacteriodes fragilis or C. perf & oral infections with mouth anaerobes like Actinomyces)

26
Q

The sulfonamides (Sulfamethoxazole, sulfisoxazole, sulfadiazine) treat?

A

Gram+, Gram-, Nocardia, Chlamydia. Can use SMX for simple UTI

27
Q

Trimethoprim treats?

A

Used in combo with SMX (TMP-SMX) for UTIs, Shigella, Salmonella, Pneumocystis jirovecii (treatment and prophylaxis)

28
Q

Fluoroquinolones (the “floxacins”) treat?

A

Gram- rods of urinary & GI tract (Pseudomonas), Neisseria, and some Gram+ organisms

29
Q

Metronidazole treats?

A

GET GAP: Giardia, Entamoeba, Trichomonas, Gardnerella vaginalis, Anaerobes (Bacteriodes, D. diff), & H. Pylori (along with a PPI and clarithromycin.. “triple therapy”)
