Brucellosis (malta fever)
mechanism of transmission
- consumption of infected ,unpasteurized animal products
*contact of skin or mucous membrane with infected animal tissue (placenta..), blood milk urine
- inhalation of infected aerosolized particles
Brucellosis signs and symptoms
*fever * malaise * night swats *arthralgia (musculoskeletal ) *hepatomegaly *splenomegaly
* osteomyelitis more common at the cervical spine than lumbar
Brucellosis laboratory findings
- elevated alanine aminotransferase
*mild anemia *leukopenia *leukocytosis *relative leukocytosis * thrombocytopenia *pancytopenia
Brucellosis tx:
adult males
doxycycline+ streptomycin
Brucellosis tx: nonpregnant adult
doxycycline +gentamicin
Brucellosis tx: children >6yo
doxycycline + rifampin
Brucellosis tx: children < 6yo and pregnant woman
TMP-SMX + rifampin
Brucellosis incubation period
from one week to several months
with symptoms appearing suddenly or gradually
primary goal of brucellosis treatment is
address current infection and prevent relapses
Brucellosis tx period of time
in nonfocal Brucellosis in adults involves 6 weeks course with at least two antimicrobial agents ,typically IM streptomycin+ doxycycline
