Bronchial carcinoma Flashcards
General features of cancer
- malignant tumour
- uncontrolled replication
- local invasion
- metastases (lymphatic spread, blood stream, serous cavity)
risk factors
- smoking
- asbestos
- radiation
symptoms
- cough
- haemoptysis
- dyspnoea
- chest pain
- recurrent/ slowly resolving pneumonia
- lethargy
- weight loss
signs
- cachexia
- anaemia
- clubbing
- supraclavicular nodes
features of small cell
- rapidly progressive disease
- early metastases
- rarely suitable for surgery
- good initial response to chemo
non-small cell
- includes squamous + adenocarcinomas
- surgery or radical radiotherapy
- less response to chemo
Metastases places
- liver
- brain
- bone
- adrenal
- skin
complications
Local:
- recurrent laryngeal nerve palsy
- phrenic nerve palsy
- rib erosion
- pericarditis
CXR
peripheral nodule, consolidation, lung collapse, pleural effusion
CT and PET scan
stage tumour
bronchoscopy
give histology and assess operability
other tests
FBC, spirometry. endobronchial ultrasound (EBUS)
Radiotherapy
small cell - used for bronchial obstruction, SVC obstruction, haemoptysis, bone pain and cerebral metastases
endobronchial therapy
small cell - stent insertion for stridor, photodynamic therapy, laser
pleural drainage
small cell - pleural effusions
surgery
non small cell
curative radiotherapy
non small cell
